Sports Clearances

MEDICAL OFFICE SPORTS CLEARANCE PROCESS

 

 

All students who intend to participate in Interscholastic Sports must:

 

 

  • Have a current physical exam on file dated within one year of the start of the sport. Please make sure your child’s pediatrician checks the box “Family Cardiac History Reviewed” on the physical exam, page 2This is in reference to the Dominic Murray Sudden Cardiac Arrest Prevent Act and is required for your child to be cleared. Click on the link for more information: NYSDOH Sudden Cardiac Arrest in Youth.  Only the required NYS form may be used (found on our website and below):

Upload the completed Physical Examination Form to the Port Washington Athletics Online Registration portal.

 

  • Complete and sign the Health Questionnaire in the Port Washington Athletics Online Registration portal. The Health Questionnaire can only be filled out within 30 days of the start of the sport.

 

  • For use of an EpiPen or an Inhaler, a Parent and Pediatrician must fill out an “Authorization for Administration of Medication in School” form (found on our website).  Parts A, B and C must be filled out.  This form is only valid for one year.  You will not be cleared without this form.

  • Please do not wait to register until the day of tryouts! You will not be cleared in time.

For your convenience, the Required NYS Physical Examination Form and Authorization for the Administration of Medication Form can be downloaded from the Portnet.org website under “Parents” tab – “Medical Forms”.

 

POST COVID CLEARANCE FOR SPORTS:

If your child has been diagnosed with Covid, please reach out to the Medical Office before your child returns to School/Sports to discuss the severity of symptoms. Depending on severity of illness, your child may or may not need clearance to return to play from a physician.

 

If your child was asymptomatic or had mild symptoms of Covid-19 (less than 4 days of fever greater than 100.4°F; less than 1 week of muscle aches and pain, chills, feeling sleepy, fatigued and/or sluggish), a clearance letter is not mandatory. Your child may return to sports after completing the 5-day isolation period. 

 

If your child had moderate/severe symptoms of COVID-19 (greater than 4 days of fever higher than 100.4°F, greater than 1 week of muscle aches and pain, chills, feeling sleepy, fatigued and/or sluggish, or a non-ICU hospital stay and no evidence of multisystem inflammatory syndrome in children [MIS-C]), your child must have clearance from a physician after 10 days from symptom start date before returning to Sports. Please have the Post Covid Return to Play Clearance filled out and signed by a physician.
